Deposits: Freedom From Pain Institute will accept a deposit of $250.00 to “hold” a reservation for the workshop. The balance of $245.00 must be paid in full by April 22, 2015. If the balance is not paid by April 22, 2015, then the registration will be cancelled and the deposit will be forfeited. It will be the duty and responsibility of the customer to contact our office to pay the balance due on or before April 22nd. We will not call to collect your balance. Deposits and balances must be paid by phone to our office at 1-800-709-5054 or 1-405-728-4844 by 5 PM central time on this date. Post-dated checks and checks from banks outside the USA will not be accepted. Foreign currency is not accepted. We accept VISA, Master Card, American Express, Discover, and PayPal.
Deposit & Regeistration Cancellation Policy: Cancellations for deposits and registrations called in to our office by April 22, 2015 will be refunded minus a $100 cancellation fee. Cancellations called in to our office between April 23, 2015 and May 22, 2015 will be refunded minus a $250 cancellation fee. After May 22, 2015, there will be NO REFUNDS.
